What are the qualifications?

You must live in West Volusia for at least three months (one month for homeless).

Your income must be below, equal or up to 150% of the Federal Poverty Level Guidelines for your family size.

How Can I Get a WVHA Health Card?

You must complete an application and provide the following documents:

Proof of residency

Two (2) forms of identification – One (1) must be a picture ID.

Proof of Income/Assets

Proof of Medicaid Application or Medicaid Application Denial Letter

Proof of Affordable Care Act (www.healthcare.gov) Application
